Special Report and Filing Requirements

The following special reports are required by the California Insurance Code or California Code of Regulations. If you have data to report for the specified years, please download the Excel Workbook from this website and use it to complete your reporting. For the Signature Page when required, you can download a copy, and have it signed and saved/scanned back to a PDF file. If you are unable to scan your signature page to a PDF, you could submit it by regular mail to the address listed below.

We are no longer requiring companies to print out the reporting forms. You can e-mail them to:

If supplemental documents are required, you can mail them to:

If you have a question specific to the data call, please e-mail it to the specific e-mail address mentioned above. You should include your name, company name, telephone number, and e-mail address in all correspondence with the CDI. Please note that data calls, 1. Survey of Transportation of Network Company Insurance Experience, 2. Credit Property & Credit Unemployment, 3. Child Care Report, 4. Legal Professional Liability Insurance Report, 5. Medical Professional Liability Insurance Report, and 6. Survey of Bail Bond Business Data Call are discontinued for now.
